Changing the name on your driver licence

You might want to change the name on your driver licence if you're:

  • getting married or being joined in civil union and taking your partner's name
  • dissolving your marriage or civil union
  • changing your name by statutory declaration (deed poll).

How do I change the name on my licence?

You will need to:

  • go to a driver licensing agent
  • take evidence of your identity (for example, your current driver licence, see Factsheet 20, Identification for driver licensing for more information)
  • take evidence of your address (for example, a recent phone or power bill, or bank statement, which has your name and current address on it).
  • take proof of your name change, which should be one of the following[1] and it must be an original document:
    • your marriage or civil union certificate
    • a dissolution of marriage or civil union order
    • change of name by statutory declaration, issued by Births, Deaths and Marriages
    • a deed poll of change of name, issued by Births, Deaths and Marriages
    • other suitable evidence of name change (eg, adoption papers)
  • fill out form DL2 (Application for a replacement licence)
  • pay the replacement licence fee of $31.10 or licence renewal fee of $44.30.

The agent may take your photo and record your signature.

If you've changed your name several times, the agent might need to see all of your original certificates and name change documents (to see a clear 'trail' from the name on your current driver licence through to the name you want to appear on the new licence).

1If the document you present as proof of name change does not show the name you wish to be known by, you will be asked to provide additional confirmation of the new name (eg, birth certificate, passport or other acceptable document).
